Nierman Preschool Required Admission Forms
In order to complete your registration for Nierman Preschool, the following forms must be filled out, printed, signed and turned in to the preschool prior to your child's start date. Children may not attend until all forms are submitted. Please return all required forms to Jennifer Meltzer | jenniferm@lfjcc.org
NEW STUDENTS ONLY State Mandated Forms Required prior to start date. Please turn in a copy of your child's immunization
record as well.
Family Information Sheet (FISH) - Required from all NEW and RETURNING students.
Allergy Forms: ONLY COMPLETE IF YOUR CHILD HAS AN ALLERGY REQUIRING MEDICATION OR OTHER TREATMENT
Parent Custody Form - Optional only if needed.
