Nierman Preschool Required Forms
In order to complete your registration for Nierman Preschool, the following forms must be filled out, printed, signed and turned in to the preschool:
Preschool Information Update: Required from all NEW & RETURNING students.
Have there been any changes to your contact information since you submitted your application? It is most important that we can reach you! Click on the link and make any changes (e.g. address, phone, e-mail, pediatrician, pick-up authorization, etc.) to your school file. You must use your JCC login information to access your account. DO NOT MAKE A NEW ACCOUNT. If you need help, please contact JCC Guest Services at (858) 457-3030.
State Mandated Forms: Required from all NEW students. Please turn in a copy of your child's immunization form as well.
Confidential Family History: Required from all NEW AND RETURNING students.
Allergy Forms: Optional, unless your child has an allergy that requires medication or other treatment.
Parent Custody Form: Optional as needed.
